Radiation Oncology Census Survey 2008


The American Society for Therapeutic Radiology and Oncology (ASTRO) has developed this questionnaire to collect information regarding the resources available to deliver radiation therapy in the United States.  The following questionnaire was drafted by the ASTRO Research Council.

Please complete the information requested to enable us to best determine the resources available for radiation therapy. Where possible, we populated the questionnaire from information obtained from our database, an article in the International Journal of Radiation Oncology, Biology and Physics*, and the public area on the Radiological Physics Center (RPC) web site.  Please review these data and make changes wherever necessary.

You may find that you will want to save your work and return to it later, or ask another staff member to contribute. The survey consists of three sections, which address demographics and equipment, staffing and reimbursement, and clinical practice patterns. We recommend you press the "Save and Exit" button, then forward our emails providing the web page link and password to your colleague. At key locations, we have provided a method to forward the questionnaire to a colleague.
